By completing the onboarding steps below, you’ll establish the legal, financial, and compliance foundation necessary for us to purchase your workers’ compensation pharmacy receivables safely and efficiently.

Follow the checklist below to complete your onboarding and activate your CompRx account.

Step 1: Review Requirements

Before submitting any forms, please confirm that you:

  • Dispense or bill for workers’ compensation prescriptions
  • Operate under a legally established business entity separate from your clinical practice name
  • Are enrolled or plan to enroll in the ExpressRx mail-order pharmacy program


If you meet these requirements, you’re ready to begin onboarding with CompRx.

Step 2: Complete Required Documents

Each onboarding item below is required before your account can be activated.

All forms should be downloaded, completed, and returned by email to info@comprx.com.

REQUIRED DOCUMENTS

  1. CompRx New Client Form
  2. Signed Non-Disclosure Agreement (NDA) and Business Associate Agreement (BAA)
  3. Healthcare Accounts Receivable Purchase Agreement
  4. LaborRx Billing and Collection Agreement
  5. ACH Authorization Form (for direct deposit)
  6. W-9 for your entity
  7. Proof of six-month enrollment with ExpressRx


These documents protect both parties and ensure compliance with federal and state healthcare laws governing data security, patient privacy, and financial transactions.
